Every VPN on this list ships a native macOS app, so the question is not whether it runs on a Mac but what you get for the money: how many devices one plan covers, what the privacy policy actually says, and what it costs once the introductory term ends.

Count your devices first. A Mac is rarely the only thing on the account. Surfshark and Private Internet Access allow unlimited simultaneous connections; NordVPN and Proton VPN allow ten; ExpressVPN eight and CyberGhost seven. If you have a Mac, a phone, an iPad and a partner's laptop, that difference decides the shortlist.
A browser extension only protects the browser. A native macOS app protects everything on the machine, which is what you want if you use Mail or any desktop app that talks to the internet. Every provider here offers the native app; the extension is an extra, not a substitute.
The protocol matters more than the interface. All of these support WireGuard or an equivalent modern protocol, which is what makes a VPN fast enough to leave on permanently. Beyond that the differences are conveniences: how quickly it reconnects after sleep, whether it starts at login, and how clearly it tells you the connection has dropped.
Almost every headline price here is a two or three year commitment. Year one is inexpensive and renewal is not. Check the renewal rate before you decide, and note that a 30-day money-back window gives you time to test on your own connection.
macOS is not immune to network snooping. A VPN encrypts traffic on public wifi and hides your IP from the sites you visit. It is not antivirus and will not stop malware you install yourself.
With a modern protocol the overhead is small, usually a few percent on a fast connection. Distance to the server matters far more than the app itself.
Treat free-only VPNs with caution. A free tier from a provider with a paid product is funded by subscribers; a free-only app has to make money another way.
